Nurse Case Manager | Onsite, Night-Shift | Up to PHP50K
GoTeam is seeking a dedicated and compassionate Nurse Case Manager to join our healthcare team. As a vital member of the care coordination process, you will play a pivotal role in developing and implementing comprehensive care plans for patients, ensuring their well-being and guiding them towards optimal health outcomes.

In this position, you will be:
- Collaborating with healthcare professionals to develop and implementing comprehensive care plans for patients, monitoring the delivery of care to align with established plans and cater to individual needs.
- Serving as an advocate for patients, respecting their rights and preferences, acting as a liaison between patients, families, and the healthcare team, and facilitating effective communication.
- Conducting thorough assessments of patients’ physical, emotional, and social needs, continuously evaluating their progress and modifying care plans to achieve optimal outcomes.
- Optimizing the utilization of healthcare resources, coordinating referrals, scheduling appointments, and facilitating access to appropriate services while ensuring timely discharge planning.
- Providing clear and concise education to patients and families about their medical condition, treatment options, and self-care strategies, empowering patients to make informed decisions regarding their health.
- Maintaining accurate and up-to-date documentation of patient assessments, care plans, and outcomes, generating comprehensive reports to comply with regulatory standards and organizational policies.
- Staying updated with healthcare policies, regulations, and evidence-based practices, contributing to quality improvement initiatives and sharing knowledge with the healthcare team.
- Bachelor’s degree or Master’s degree in Nursing or a related field is highly preferred.
- Active Registered Nurse (RN) license in good standing is a mandatory requirement.
- Minimum of 3-5 years of progressive clinical nursing experience, with a preference for experience in a hospital or acute care setting.
- Comprehensive knowledge of clinical best practices, care coordination principles, and healthcare systems.
- Excellent communication and interpersonal skills, adept at building rapport with patients, families, and interdisciplinary healthcare teams.
- Proficient in utilizing electronic health records (EHR) and other advanced healthcare software applications.
- Exceptional organizational and time-management skills, demonstrating the ability to prioritize and manage multiple cases effectively.
- Demonstrated critical thinking and problem-solving abilities, allowing for sound decision-making in complex healthcare scenarios.
- Familiarity with regulatory guidelines and reimbursement processes related to case management.
- Certification in Case Management (CCM) or similar credential is a plus.
